wiring issue - only left channel plays - volume control

Hi,

I've installed new ceiling speakers. I have a left and a right channel.

Wiring: Amp (4 conductor) -> Volume Control Switch -> L/R individual cables

I can only get the left channel to play. It doesn't matter which speaker, it will only play if plugged in to left channel of amp (Onkyo). I have tested the right channel on the amp to a direct speaker and it works. I've also tried using B speakers. I have also tried a different volume control switch.

Volume control order:

AMP SPEAKERS

L+ L- R- R+ | L+ L- R- R+

Red black green white (amp)

I've triple checked all the polarity matches. Tried swapping just in case. Nothing seems to make a difference, but as I have ruled out all hardware, I feel like it has to be a wiring issue of some type.

Anyone have any ideas?
